Mediation Intake Packet

Both parents must complete, electronically sign, and submit the intake form below before a mediation appointment can be scheduled.

Before your appointment

1. Submit the Mediation Intake Packet using the form at the bottom of this page. Each parent submits their own.
2. Complete mediation orientation.

Please note: Voluntary mediation is available once per year. If you need changes to your custody agreement within a year of your voluntary mediation date, you will need to file the appropriate paperwork to have your case placed on calendar for judicial review. See Local Rules of Court, Rule 12.1(B).
